As far as I can figure out, it's clan tier based. I've yet to see a higher tier companions at the beginning and I've tried to get them pretty early on usually. I always go straight to the best at clan level 0 and while some are were the map says they are, most aren't. By clan tier 3, all are, so hence my theory. Could be player lever driven though
My companion's squad with 275 tactics and perks , 230 elite units fought an enemy army with 350 units. My companion won, he has 163 units left. After that I load save again, and using the console, reset tactics to 0 and all perks. And repeate fight. Now it was much longer, even allied lord with 100 units arrived to help.
The easiest way to level steward is coincidentally the easiest way to level leadership: form an army. Join a kingdom, grab a companion, create a party with that companion, and then create an army with that companion. Now just go about your normal play. If cohesion gets low, you can raise it for free via the army menu since the army itself costs

On the whole, I think glaives are the best. Better yet if you do some smithing to make a 2H polearm that is swing-only, with the blade as long as possible, but shortened handle (so that it swings faster). Basically a shortened, faster glaive. Well I usually got a shield wall to place my guys behind.
Beyond that I generally favour low level relatively young (as I like long campaigns) companions with good ranged skills and two handed, so swift, wainwright, vlandian black, empire hills and frostbeard are all useful. The absolute best companions in my opinion are 'kidnapped daughters' if you don't mind failing a quest.
I got extremely lucky and was given a town and castle that I had no part in taking. You can then let your castle/town build up militia, and build up its garrison yourself, then leave the kingdom and keep the stuff you hold. Clan>Parties>Create new party. You can then add them to your army, if you're a kingdom.
